Outpatient/Ambulatory - Woodland Hills

Welcome to Kaiser Permanente Woodland Hills Medical Center!
Please ensure to complete the Student and/or Faculty Prerequisites for Kaiser Permanente (KP) Unpaid Field Experience and Training from the Nursing Pathways main web page and the following:

1. Student Placement Request
   To begin the clinical request process, the Student Placement Request must be submitted 3 months prior to desired start date. Requests are processed on a first-come, first-serve basis and is also based on space availability. Additionally, please submit a copy of your syllabus and objectives with your request.
2. Pre-Clinical Paperwork
   Due minimum 2 weeks prior to requested start date. Please note, only complete packets submitted electronically, as a single PDF file will be processed. Partial packets will NOT be accepted. Failure to submit on time will result in request being denied. Please click on appropriate checklist below to ensure additional items are submitted for our medical center.

4. LA city Fire Card
   Required if clinicals will be completed at the Woodland Hills Medical Center (excludes outlying clinics such as Market Street, Porter Ranch, etc.)
