  11. Health screening for students (to be completed 1-2 weeks prior to start of rotation)
    • School Coordinator to complete the Health & Safety Verification Form (located under “Clinical Coordinator/Designee” on previous page).
    • Academic partner to email forms per clinical group to employee-health-services-BPK@kp.org and please CC the Academic Liaison.
    • On subject line of emails, enter: (PHI), name of the academic institution, and clinical rotation/group 
    • A reply email will be sent informing that the students' health clearances are completed, or what corrections/additional information is required. 
    • When cleared, clinical instructors must submit badge clearance forms to employee health in person.
  12. ROCHE Glucometer Competency Form
    • Faculty and students must first complete the KP Learn Module on StatStrip Training.
    • Faculty will be trained on Nova StatStrip as part of the Unit Specific Orientation.
    • Faculty will complete students' training and competency. Please make a photocopy of the completed competency.
    • Faculty will deliver original completed competencies directly to the Laboratory Point of Care Supervisor and provide photocopies to the Unit Clinical Nurse Educator.
    • Laboratory is located on the 1st floor of the Medical Center building.
  13. Badges
    Students to complete the badge form, and to give the form to the clinical instructor. Instructor to take the badge form to Employee Health. When instructor gets clearance for all the students, Employee Health will sign the badge forms. Instructor will also get the Clinical Department Manager's signature before giving the badge form back to the students. Students to take the ID badge form to Security Department located on 1st floor in Staff Parking Structure, to have their picture taken and to get a badge.
