Application:
Unpaid field experience and training program placement is based on space availability and at the discretion of the Department Administrator and/or Director of Education.

To begin the clinical placement process, Individual Preceptorship Students and Faculty must complete the Unpaid Field Experience and Training Application Request

Section I: All Students and Faculty

1. CPMKPHC Student-Instructor Access Data Spreadsheet = KP Computer Access Request (National Users Identification Number (NUID)/HealthConnect Access) (Complete & Submit at least 4 weeks prior) 
   Note: For Student Cohorts/Groups this form will be completed by your Faculty
2. Panorama City Prerequisite Packet Checklist (Review)
3. Emergency Codes and Alerts (Read)
4. KP Learn (web based learning) Instructions & Required Courses for Students/Faculty (Complete & Submit)
5. KP PC Infection Prevention
1. Infection Prevention Student Module (Read)
2. Infection Prevention Attestation (Print & Complete)

Section II. Diagnostic Imaging (DI) Students Only
Students in a Radiologic Technology, Sonography or Nuclear Medicine Program.

1. Age Specific Competency Critical Elements (Read)  
2. Age-Specific Characteristics Competency Evaluation Post Test (Print & Complete)
3. Restraint or Knot Review and Post Test (Read, Print & Complete)
4. Needle Stick & Sharps Exposure Prevention and Post Test (Read, Print Test & Complete)
