2. DMC Safety Attestation form: Please print, sign and return.
3. Safety Verification Record: Individuals please complete. Instructors please complete one form per group.
4. Badges
   This form requires a Department Manager signature. The Medical Center Specific Academic Liaison or the Department Manager can sign this form. Please contact Rosette.R.Martinez@kp.org for details. After you have the Department Manager signature, please take the form to Security Department located in Orchard Parking Structure to have your picture taken and to get a badge.
5. Health Screen
   School to complete Attestation Form for each student and instructor and fax to Employee Health (fax #: 562-461-6520) at least 2 to 3 weeks prior to the start of your rotation.

6. Competency Validation
• Pyxis
  (Only instructors are given Pyxis access. Please contact Rosette.R.Martinez@kp.org for details)
• Nova Statstrip Competency
  As a part of your Unit Specific Orientation, you or your group may make arrangements to be validated to use the Nova Statstrip system to complete blood sugar checks on the unit.  Validation will be conducted in the Laboratory, please contact Jonathan.D.Rabot@kp.org or Barbara.A.Hickey@kp.org for more information.
7. General Information
   
• Parking Map
  Monday - Friday students and instructors are to park at the Kaiser Satellite facility (12200 Bellflower Blvd). A shuttle runs from 0600-0900 and from 1600-1800. If you are working a 12-hour day, please make arrangements to move your car in the late afternoon (to the Promenade Lot located at Steve Horn Way/Bellflower Blvd or walk to the parking lot after your shift (approx .75 miles).

• To request conference rooms at Kaiser Downey, please complete the following form and e-mail to Julia.X.Padilla@kp.org. Instructors: you may book a conference room for 30-60 minutes both at the beginning and end of your shifts.

8. Clinical Placement Orientation

   A current KPSC School Affiliation Contract Agreement with Kaiser Permanente is required for clinical placements. Faculty and students must complete all orientation requirements prior to the start of any rotation at a Kaiser Permanente facility.
