After you have the Department Manager signature, please take the form to Security Department located in Orchard Parking Structure to have your picture taken and to get a badge.
Health Screen
School to complete Attestation Form for each student and instructor and fax to Employee Health (fax #: 562-461-6520) at least 2 to 3 weeks prior to the start of your rotation.
A current KPSC School Affiliation Contract Agreement with Kaiser Permanente is required for clinical placements. Faculty and students must complete all orientation requirements prior to the start of any rotation at a Kaiser Permanente facility.
